Registration Checklist
At the time of registration, you will be required to bring the following documentation:

• Proof of Age:  Birth Certificate, Hospital Certificate (must have attending physician's signature), Baptismal Certificate, Passport or affidavit of parent, guardian, or custodian of the minor.
• Proof of Residency:  Current utility bill such as:  electric, water, or gas bearing the parent's name and address (bill must be 30 days old or less)
• Proof of Immunizations:  Current immunization record
